Application Forms - (for FYP students)
A number of forms are available in the laboratory office for applying network account, locker, and magnetic access card. All these forms can also be downloaded from here:

Note:
- Please read the regulation policy for the use of facilities and services in the Lab. (HTML or Word)
- Valid entry for
Card Access System: Monday to Sunday : 8:30am -- 11:00pm (include holiday)
** All undergraduate students must leave before 11:00pm - All forms have to be signed for the approval by supervisors.
